Mellow Care is a WA DOH-licensed home care agency serving King, Pierce, and Snohomish Counties. We accept referrals from hospitals, skilled nursing facilities, care managers, and assisted living communities.
Use the form below, call our Administrator directly, or fax discharge paperwork. We accept referrals for personal care, homemaker support, companionship, and respite.
Our Administrator reviews the referral against staffing and service-area capacity and confirms acceptance — or is upfront if it's outside our scope.
We schedule the on-site assessment, build the Plan of Care, and keep you updated on start-of-care status — care begins within seven calendar days.
No call center. Referral status, scheduling questions, and concerns go straight to Eline, our founder and Administrator.
Licensed personnel, background-checked caregivers, and a documented Quality Improvement program — ready for your own compliance review.
If a discharge or safety concern arises, we self-report to DSHS or the Area Agency on Aging as appropriate — you're never left guessing.
